Immunization Requirements:

You will be required to provide proof of current immunizations for MMR, Varicella, Tdap, HepB, and a negative TB test. The complete immunization document must be emailed to marianne.graham@dallascityhall.com at least 3 weeks prior to the class date or you will be cancelled from the class.
