Immunization Requirements:

Proof of the following vaccinations are required to attend at Mom's Place. If you have any questions please call Susan Glaeser RNC, BSN, IBCLC at Mom's Place 512-972-6700. Titers proving immunity will suffice if you do not have your immunization records. Immunity to many of the diseases can be detected by blood sample at your doctor's office: Measles Mumps Rubella, Hepatitis B, Varicella (chicken pox).

Measles: Vaccine record of two doses separated by 28 days on or after the 1st birthday or serologic evidence of immunity to mumps.

Mumps: Vaccine record of two does separated by 28 days on or after the 1st birthday or serologic evidence of immunity to measles.

Rubella: Vaccine record of one dose of rubella on or after the 1st birthday or serologic evidence of immunity to rubella.

Varicella: Vaccine record of two doses of varicella given at least 28 days apart or written physician diagnosis of varicella or herpes zoster or serologic evidence of immunity to varicella.

Tdap: One dose of Tdap Adult Formulation Vaccine

Hepatitis B: Vaccine record of 3 doses of HepB vaccine or serologic evidence of immunity or signed Hepatitis B Vaccine Declination Form

TB Skin Test: Annual skin test or quantiferon (QTF) testing
